Fewer than 20 teeth tied to 52% higher weight-loss risk

Pooling 19 studies of older adults, having fewer than 20 teeth was associated with a 52% higher risk of unintentional weight loss, and being edentulous with a 43% higher likelihood of malnutrition risk. People with 20 or more teeth consistently had higher intakes of a range of nutrients.

Why it matters

Older adults are broadly at risk of malnutrition as physical function declines, with or without systemic disease, and identifying modifiable contributors is a priority in geriatric care. Chewing is the first step of eating, so the number of remaining teeth and the presence of occlusal support — opposing teeth that meet to allow effective chewing — is a plausible determinant of what people can and do eat. Fibrous foods such as vegetables are the hardest to manage with compromised dentition. Whether that translates into measurable nutritional harm, rather than just dietary preference, is the question this review set out to answer.

What they did

Following PRISMA guidelines and pre-registered on PROSPERO, the authors searched PubMed, Web of Science, EMBASE, Google Scholar and the ICHUSHI database between July and September 2024. Eight independent researchers carried out data extraction and quality assessment using the modified Newcastle-Ottawa Scale. Nineteen studies met inclusion criteria — 12 cross-sectional and 7 cohort studies — examining the association between dentition and occlusal support measures and nutritional conditions in older adults. Where data allowed, results were pooled into meta-analyses, though the number of studies contributing to each pooled estimate was small.

What they found

Across the included studies, individuals with 20 or more teeth consistently had significantly higher intake of various nutrients than those with fewer teeth, and compromised occlusal support was associated with reduced intake of vegetables, dietary fibre and vitamins. Several studies showed a significant association between edentulism or absent occlusal support and increased malnutrition risk. A meta-analysis of two studies found that having fewer than 20 teeth carried a 52% increased risk of unintentional weight loss (OR 1.52; 95% CI 1.08-2.14). A separate meta-analysis found edentulous individuals had a 43% higher likelihood of malnutrition risk than dentate individuals (OR 1.43; 95% CI 1.13-1.80).

What it actually shows

Systematic review of 19 observational studies (12 cross-sectional, 7 cohort), with the headline pooled estimates resting on just two studies each; observational data cannot show that tooth loss causes malnutrition rather than tracking shared frailty.

Where it fits

The direction of the finding is consistent across 19 studies, which is reassuring, but the two pooled estimates that generate the headline numbers each rest on a very small number of contributing studies. Twelve of the nineteen were cross-sectional, meaning diet and dentition were measured at the same moment, so they cannot establish which came first. Tooth loss also tracks with broader frailty, socioeconomic position and chronic disease, any of which could independently drive poor nutrition. The authors' conclusion is that oral health assessment deserves integration into routine geriatric care rather than that fixing teeth would fix nutrition.

What it means for you

This is a reason to think of dental status as part of the nutrition picture in later life rather than a separate cosmetic concern. The recurring threshold across the literature is 20 teeth, above which nutrient intakes were consistently higher, and the shortfalls concentrated in vegetables, dietary fibre and vitamins — the foods that require the most chewing. Because the evidence is observational, it shows association rather than proof that losing teeth causes malnutrition. It does make a case for why oral health checks feature in assessments of older adults.
