Fermented Foods: What One Good Trial Showed

A genuinely interesting randomised trial, and an honest account of how much it proves.

What this is

Fermented foods are the rare wellness recommendation with a decent randomised trial behind it. Stanford researchers randomised healthy adults to ten weeks of either a high-fermented-food diet or a high-fibre diet, and measured the microbiome and immune markers.

The fermented-food arm increased overall microbial diversity and reduced several inflammatory proteins, with larger servings producing larger effects. The high-fibre arm — to the researchers' surprise — did not increase diversity over that period, though it changed other measures.

    They were part of the trial's food list, not tested individually, and many commercial versions carry a lot of sugar. Treat as a pleasant drink with a plausible upside.

What this means in practice

  • Include a live-culture food most days — plain yoghurt, kefir, kimchi, sauerkraut, miso.
  • Check for "live cultures" and avoid pasteurised-after-fermentation products if the microbiome is the point.
  • Build up gradually; wind and bloating are common at first.
  • Keep the fibre target as the main event. Fermented foods are the promising extra, not the replacement.

The honest summary

One good trial, a plausible mechanism, cheap and enjoyable to act on. That combination earns a place in the diet without earning any of the claims on the bottles.

They're differently evidenced. Fermented foods have this trial plus a long observational history; specific probiotic strains have evidence for specific clinical conditions. Neither supports general-purpose probiotic capsules for healthy people.
