Fasted workouts show no metabolic edge over fed ones

Why it matters

Fasted cardio has a devoted following built on a simple pitch: train before eating and your body, deprived of fresh fuel, will handle fats and glucose more favourably. Exercise in the fasted state is frequently promoted as producing superior metabolic adaptations compared with training after a meal. Yet the evidence behind that pitch has been scattered across many small trials measuring different markers in different ways. This review set out to pool those trials and answer a basic question: when healthy adults perform a single bout of exercise fasted rather than fed, what actually happens to their glucose, insulin and blood fats?

What they did

The authors systematically searched PubMed, Scopus and Web of Science up to July 2023 for randomised clinical trials comparing single bouts of exercise performed in fasted versus fed states in healthy adults. Eligible trials measured serum glucose, insulin, triacylglycerol, free fatty acid concentrations, or the respiratory exchange ratio, a marker of which fuel the body is burning. Twenty-eight studies met the criteria, covering 302 healthy adults in total, with exercise sessions lasting between 36 and 150 minutes. The team then pooled the changes from before to after exercise as weighted mean differences to compare the two feeding states.

What they found

Compared with fed exercise, fasted sessions were associated with larger increases in serum glucose, by 0.263 mmol/L, and insulin, by 8.84 mU/mL, from before to after exercise. Free fatty acids fell further in the fasted state, by 0.121 mmol/L. Two markers did not budge either way: changes in triacylglycerol and in the respiratory exchange ratio were no different between fasted and fed exercise. The authors conclude that the results do not suggest fasted exercise is necessarily better for glucose or lipid metabolism, and note it is possible, although in their view unlikely, that fasted bouts cause some degree of metabolic impairment.

Where it fits

The result pushes back against a popular belief that training on an empty stomach delivers a superior metabolic stimulus. The larger fall in free fatty acids shows the fasted state does change fuel handling, yet the unchanged respiratory exchange ratio complicates any simple fat-burning story, and the larger rises in glucose and insulin sit awkwardly beside the claimed benefits. Because every included trial examined single exercise bouts, the bigger question remains open: whether weeks or months of habitual fasted training produce different adaptations in body composition, metabolic health or fitness than the same training done fed.

What it means for you

If you train before breakfast because it suits your schedule or your stomach, nothing here counts against that. But if the reason is a supposed metabolic advantage, this pooled evidence offers it little support: single fasted sessions did not improve the glucose or lipid response compared with fed ones, and on some markers the pattern ran the other way. That is a reason to view fasted cardio as a personal preference rather than a metabolic strategy. What long-term fasted training does over months, if anything different, is a question this analysis was not designed to answer.
