Why it matters

L-arginine is an amino acid sold widely as a pre-workout ingredient and often pitched for cardiovascular health. Several trials have tested whether supplementing it improves blood lipids such as cholesterol and triglycerides, or dampens inflammatory markers, but their findings have pointed in different directions. When single trials disagree, pooling them is the fairest way to judge whether an effect exists at all. This analysis set out to settle what oral L-arginine actually does to lipid profiles and inflammation across the randomised evidence.

What they did

The researchers searched PubMed/Medline, Scopus, ISI Web of Science and Google Scholar for randomised controlled trials published up to May 2019. From 6282 records identified, 17 trials met the criteria and were included. They pooled the mean changes in lipid profiles and inflammatory markers between L-arginine and control groups using a random effects model, and ran subgroup analyses to hunt for reasons the trials might differ from one another.

What they found

Across the 13 trials reporting triglycerides, L-arginine produced a reduction of 6.03 mg/dl on average, a result the authors describe as borderline significant. Everything else was flat: there was no significant effect on LDL cholesterol, total cholesterol or HDL cholesterol compared with placebo. The inflammatory markers CRP and TNFα were likewise unmoved. So of the outcomes examined, only one shifted, and only just.

Where it fits

This pooled picture cools the enthusiasm around L-arginine as a lipid or inflammation tool. Individual trials had reported inconsistent findings, and this analysis suggests the positive ones were the exception rather than the rule. A triglyceride reduction of 6.03 mg/dl is small in practical terms, and its borderline statistical status means even that could be fragile. The authors call for further randomised trials on other lipid and inflammation measures, leaving the door ajar without offering much through it.

What it means for you

If L-arginine sits in your supplement stack for pump or performance reasons, this analysis says nothing about those uses either way. What it does say is that buying it to improve your cholesterol numbers or lower inflammation is not backed by the pooled trial record. The one measurable effect, a modest dip in triglycerides, is small enough that everyday diet and exercise choices would dwarf it. Knowing which claims a supplement cannot support is half the value of reading the evidence.
