Why it matters
The longevity case for aerobic exercise is well established, but resistance training has never enjoyed the same consensus. Public health guidance increasingly recommends lifting alongside cardio, yet whether picking up weights is actually associated with living longer, and whether it adds anything on top of aerobic work, had not been settled. This systematic review and meta-analysis set out to pool everything available on resistance training and major outcomes: death from any cause, cardiovascular death and cardiovascular events.
What they did
The researchers searched for randomised trials and cohort studies evaluating the association between resistance training and mortality or cardiovascular events. From 1430 studies identified, 11 met the inclusion criteria: one randomised trial and 10 cohort studies, together covering 370,256 participants followed for a mean of 8.85 years. Two investigators screened studies independently and in duplicate, risk of bias was assessed with Cochrane tools, and hazard ratios were calculated using random effects models.
What they found
Compared with no exercise, resistance training was associated with 21% lower all-cause mortality on its own. Combined with aerobic exercise, the association strengthened to 40% lower all-cause mortality, suggesting the two forms of exercise add to each other. The association with cardiovascular mortality was borderline rather than clear, and there was no significant association with cancer mortality. One cohort study of men found resistance training linked to a 23% lower risk of coronary heart disease events. Risk of bias in the included studies was low to intermediate.
Where it fits
This analysis gives resistance training some of the mortality evidence that aerobic exercise has long had, and it supports current guidance to do both rather than choose one. The additive pattern is the most striking element: lifting appears to bring benefit beyond what cardio alone provides. The authors are explicit about the gaps, though: there is insufficient data on non-fatal events, and nothing here says whether swapping cardio for lifting entirely would preserve the benefit. With 10 of 11 studies observational, causation remains unproven.
What it means for you
This is a reason to view strength work as more than a body-composition tool: the people who lifted, and especially those who lifted and did aerobic exercise, were considerably less likely to die over roughly nine years of follow-up. It is equally a reason not to read the numbers as a guarantee, because people who train may differ from those who do not in many ways that also affect lifespan. The most defensible reading is that lifting and cardio together kept the strongest company with long life.
