Why it matters
Caffeine is one of the best-supported performance aids in sport, with solid evidence for aerobic endurance, strength, power and muscle endurance. But most of that evidence comes from controlled physical tests, and intermittent sports such as football, tennis or judo are messier: performance depends on decision making, repeated high-intensity efforts and executing skills under pressure. Whether caffeine's gym-tested benefits actually translate into better actions during a match has been studied far less. This review set out to pool that competition-specific evidence properly for the first time.

The authors ran a systematic review and meta-analysis of studies examining acute caffeine intake and sport-specific actions in intermittent sports involving decision making and high-intensity efforts, covering team, racket and combat sports during real or simulated competition. All included studies used blinded crossover designs, meaning each athlete competed both with caffeine and with a placebo without knowing which they had taken. In total 24 studies were included, a risk-of-bias analysis was conducted, and effects were pooled with a random-effects model as standardised mean differences.
What they found
Caffeine increased a range of high-intensity actions during competition. The pooled effects covered the number of sprints (0.48), body impacts (0.28), accelerations (0.35), decelerations (0.63) and high-intensity offensive efforts (0.36). Beyond raw physical output, caffeine also improved the quality of play: athletes had a higher positive or success rate of actions during real or simulated competition (0.44). These are small to moderate standardised effects rather than transformative ones, but they were consistent in direction across the different action types measured.
Where it fits
This extends caffeine's evidence base from the laboratory into the game itself, which is the setting athletes actually care about. It suggests the well-documented physical benefits do carry over into more sprints and harder efforts when tactics, opponents and skill execution are all in play, and even into a higher success rate of actions. Open questions remain about which sports benefit most, how doses and timing matter, and whether decision making itself improves or simply the physical capacity to act on decisions. Individual variation in caffeine response is also not addressed here.
What it means for you
If you play an intermittent sport, this is a reason to think caffeine's benefits are not confined to gym tests: in pooled blinded studies, athletes produced more high-intensity actions and succeeded more often with caffeine on board during competition. The effects were modest, so caffeine will not transform a performance, and responses differ between people. How caffeine interacts with your sleep, timing of matches and personal tolerance is a separate consideration this analysis does not cover.
