Why it matters
Hard training generates a burst of free radicals, and the resulting oxidative stress is thought to interfere with several neurophysiological processes and chip away at exercise performance. Molecular hydrogen has attracted attention because it appears able to selectively neutralise excessive free radicals, and it is now sold as hydrogen-rich water, hydrogen gas and even hydrogen baths. The catch is that previous studies reported dual effects, with hydrogen sometimes raising and sometimes lowering markers of oxidative stress. This review set out to pool the randomised trial evidence and establish what hydrogen actually does in healthy adults after exercise.
What they did
The authors swept five databases for randomised controlled trials testing hydrogen against placebo for exercise-induced oxidative stress in healthy adults. Six studies made the cut, covering seven experiments and 76 participants in total. Hydrogen was delivered as hydrogen-rich water, hydrogen bathing or hydrogen-rich gas, with timing that varied from before, during or after exercise to repeated dosing over several days, and doses that ranged from single to multiple. The pooled outcomes were a blood marker of oxidative stress, d-ROMs, and a measure of antioxidant potential called BAP, with subgroup analyses probing how intervention design shaped results.
What they found
On the headline measure, hydrogen did nothing: pooled across the trials, d-ROMs after exercise were essentially identical with hydrogen and with placebo. Antioxidant potential told a different story, with hydrogen producing a small but statistically significant improvement in BAP compared with placebo. Subgroup analysis sharpened the picture, showing the antioxidant benefit was clearer for intermittent exercise than for continuous exercise. The authors conclude that hydrogen may enhance antioxidant capacity without directly lowering the oxidative stress that exercise produces.
Where it fits
This is a quantitative pooling of a small and scattered literature, and it partly resolves the confusion over hydrogen's dual effects. The claim that hydrogen directly blunts exercise-induced oxidative stress does not hold up in the pooled data, while a more modest antioxidant capacity signal does. With only 76 participants across six studies, and forms, doses and timing varying widely, the evidence base remains thin. The authors explicitly call for more rigorously designed studies, and questions about optimal form, dose and timing are wide open.
What it means for you
Hydrogen water and related products are marketed hard on recovery and oxidative stress, and this pooled analysis is a reason to treat the central claim with scepticism, since the marker of oxidative damage itself did not move. There is a genuine, if small, signal on antioxidant potential, most visible around stop-start exercise, so the story is not entirely empty. With so few participants studied, though, this is grounds for curiosity rather than confidence, and the more rigorous trials the authors call for will decide whether that signal survives.
