Why it matters

Cold water immersion is one of the most widely used post-exercise recovery tools, from club changing rooms to home tubs, yet the way people actually do it varies enormously. Temperature and duration are chosen largely by habit or folklore, and it has been unclear whether either matters or whether the practice beats simply resting. Soreness is the outcome most users care about, because it is what they feel the next morning. Establishing whether there is a dose-response relationship is the difference between a technique and a ritual.

What they did

The authors searched MEDLINE, EMBASE, SPORTDiscus, PEDro and the Cochrane trials register up to January 2015 and included only randomised controlled trials comparing cold water immersion with passive recovery. Nine studies met the criteria and were pooled in a meta-analysis, with results expressed as weighted mean differences and 95% confidence intervals. Effects were analysed separately as immediate and delayed, and the pooled data were broken down by water temperature band and by immersion time to look for a dose-response pattern.

What they found

Cold water immersion had a more positive effect than passive recovery for both immediate soreness (mean difference 0.290) and delayed soreness (mean difference 0.315), with both results statistically significant. Breaking the data down, water between 10 and 15 °C gave the best immediate (0.273) and delayed (0.317) results, and immersion of between 10 and 15 minutes performed best immediately (0.227) and later (0.317). The authors concluded that a dose-response relationship was present and that water of 11 to 15 °C for 11 to 15 minutes gave the best outcomes. They describe the overall benefit over passive recovery as slight.

Where it fits

This supports cold water immersion as a mild soreness management tool rather than a transformative one, and it puts numbers on the protocol question that most previous work left open. The small pooled effect sits alongside a wider literature in which cold exposure after exercise has been questioned on other grounds, and this review does not address adaptations, strength or performance at all. With only nine trials feeding the analysis, the temperature and time bands rest on limited data. Whether colder or longer exposures do anything different, and whether the effect holds in trained athletes specifically, remain unanswered here.

What it means for you

If you use cold water immersion for soreness, the pooled evidence says you are getting something, but not much, compared with just resting. The temperature and duration bands that performed best in this analysis were moderate rather than extreme, which is a reason to be sceptical of the idea that colder and longer must be better. Nothing here speaks to whether cold exposure helps or hinders training adaptations, so that trade-off is not settled by this review. For someone who finds the practice pleasant or useful psychologically, that remains a perfectly reasonable basis for doing it.
