Why it matters
Depression is more than low mood: repetitive negative thinking (rumination) and difficulties with executive functions such as switching between tasks are considered central features of the condition. Exercise is regarded as an effective treatment for depression overall, yet whether it reaches these specific features — and what kind of exercise, at what dose — has remained unclear. Answering that matters because a prescription that lifts mood might still leave the thinking patterns that sustain depression untouched. This review set out to test the effects of chronic exercise on emotion regulation and executive function in adults with depression, and to examine how exercise prescription characteristics moderate them.
What they did
The authors systematically searched two Chinese and two English databases for randomised controlled trials published between 1 January 2000 and 30 October 2023. Separate meta-analyses were run for each outcome: rumination (260 participants across 5 studies), inhibition (578 participants in 7 studies) and updating (832 participants in 9 studies) using random-effects models, and shifting (802 participants in 8 studies) using a fixed-effects model. Moderator analyses then probed whether exercise intensity, session duration, weekly frequency and programme length shaped the results.
What they found
Exercise improved rumination (SMD -0.59, p = 0.02), but subgroup analysis suggested this benefit was confined to moderate-intensity aerobic exercise performed in one or two 31-60 minute sessions per week for 5-8 weeks. Shifting also improved (SMD -0.22, p = 0.002), with substantial gains observed only from high-intensity aerobic exercise in three or four 31-60 minute sessions per week for more than 8 weeks. By contrast, inhibition (SMD -0.21, p = 0.18) and updating (SMD 0.15, p = 0.14) showed no significant improvement. The picture is therefore selective rather than across-the-board.
Where it fits
This extends the exercise-and-depression literature beyond overall symptom scores to the specific central features of the condition, and it hints that different symptoms may respond to different exercise prescriptions. That symptom-specific dosing idea is novel and, if replicated, would complicate one-size-fits-all exercise recommendations for depression.
