Why it matters

Resistance training is the standard route to muscle growth, but voluntary loaded movement is not available to everyone. That has driven interest in passive alternatives, in particular pairing blood flow restriction with neuromuscular electrical stimulation so that the muscle is contracted and the limb is occluded without the person lifting anything. Individual reports have claimed hypertrophy from this combination, yet protocols differ widely and the reported amount of growth varies. This review set out to establish whether the pooled evidence actually supports the combination over blood flow restriction alone, stimulation alone, or doing nothing.

What they did

The authors ran a PRISMA-compliant systematic review and meta-analysis, searching PubMed, MEDLINE, Web of Science, Scopus and CINAHL from inception to 28 February 2025. Eligible studies had to involve untrained healthy adults aged between 18 and 64, allow a comparison between the combined intervention and a control of blood flow restriction alone, stimulation alone or no intervention, measure lower-limb muscle hypertrophy before and after, and run for at least 14 days. Standardised mean differences were calculated for each comparison and combined using a multilevel random-effects model. Secondary aims covered how measurement device choice affected hypertrophy outcomes and what protocols had been used.

What they found

From 615 articles screened, only three studies met the criteria, with a combined population of 37 people, across which seven meta-analyses were run. The combined intervention produced a medium pooled effect size of d = 0.64 in its favour, but the 95% confidence interval ran from -0.37 to 1.65 and the result was not statistically significant (p = 0.176). In other words, the direction of the estimate is encouraging while its precision is poor, and the interval comfortably includes no benefit at all. The authors themselves frame the conclusion as a possible medium positive effect on lower-body muscle rather than a demonstrated one.

Where it fits

This tempers the enthusiasm generated by individual trials suggesting a passive route to hypertrophy. Rather than contradicting those reports, the pooled analysis shows how thin the evidence base is: three studies and 37 participants cannot settle the question either way, and the wide confidence interval is the honest signal here. The review also leaves the practical questions open, including which protocol variables matter, whether the same applies to upper-body muscle, and whether adolescents or older adults respond differently. Larger randomised trials are explicitly called for.

What it means for you

If you have seen the pairing of occlusion cuffs and electrical stimulation sold as a way to build muscle without training, this is a reason to treat that as unproven rather than established. The pooled estimate leans positive, which is a reason for researchers to keep looking, but it is not a result you can bank on. For anyone able to train voluntarily, nothing here displaces conventional resistance work as the better-evidenced option. The interesting open question is whether the approach eventually helps people who cannot load a limb in the usual way.
