Why it matters

Broken sleep after having a baby is close to universal, but for many mothers it tips into genuinely poor sleep quality that can harm maternal and family health. Sleeping pills sit uneasily alongside breastfeeding and newborn care, so drug-free approaches are the preferred route. Plenty of individual options exist, from massage to structured psychological therapy, yet almost nothing was known about how they compare with one another. What they did

The authors ran a systematic review and network meta-analysis, a technique that ranks treatments against one another even when they were never tested in the same trial. They searched electronic databases from their inception to July 2024 for randomised controlled trials of nonpharmacological interventions targeting postpartum sleep. Nineteen trials involving 1,826 women qualified, between them covering 13 different interventions. Effects on sleep quality were compared against usual care, analysed separately for the first 3 months after birth and for 4 to 12 months postpartum.

What they found

In the first 3 months postpartum, foot reflexology showed the largest improvement in sleep quality relative to usual care, followed by progressive muscle relaxation and then massage. Later in the first year, from 4 to 12 months, cognitive behavioural therapy for insomnia came out on top, with light-dark therapy also proving effective. The overall conclusion was that nonpharmacological interventions do improve postpartum sleep, but which one looks best appears to depend on the stage.
