Why it matters

Fibromyalgia brings widespread pain and fatigue, and exercise is one of the few non-drug strategies consistently recommended for managing it. The awkward gap in the evidence has been comparative: very few studies have set different types of exercise against each other, so patients and clinicians have had little basis for choosing between an aerobic programme, a weights programme or gentler mind-body practices. This review pooled the randomised trial evidence to compare three exercise types, alone and in combination, on the outcomes that matter most in fibromyalgia: overall impact and fatigue, pain, and physical capacity.

What they did

The authors searched PubMed, Cochrane, Embase and Web of Science from inception to October 15, 2022, for randomised controlled trials of exercise in people with fibromyalgia. Twenty-one trials with 1638 participants qualified, covering five distinct interventions built from aerobic exercise, resistance training and mind-body exercise. Most participants were female, consistent with who actually develops the condition. A network meta-analysis compared the interventions against routine healthcare on the Fibromyalgia Impact Questionnaire, a visual pain scale, and the six-minute walk test.

What they found

On pain, the largest improvements were associated with mind-body exercise (an effect of 1.62 standard deviations) and resistance training (1.58), with the combination of aerobic and mind-body exercise also showing a significant benefit (0.78). On the fatigue-oriented impact measure, the combination of aerobic and mind-body exercise, aerobic exercise alone and mind-body exercise alone were each associated with relief. Mind-body exercise also helped people walk farther on the six-minute test, and aerobic exercise showed a significant walking effect too, whereas the combination of aerobic and resistance exercise produced no significant change in walking distance.

Where it fits

The review supports exercise as a highly effective non-pharmacological strategy for fibromyalgia, which matches existing guidance, but its real contribution is the comparison between types. The strong showing of mind-body exercise on both pain and walking capacity is notable for a condition where hard training can be difficult to tolerate. The caution is that network comparisons of this kind are partly indirect, some of the pooled figures are inconsistently presented in the report, and the trials themselves varied, so the rankings should be read as tentative rather than definitive.

What it means for you

If fibromyalgia is part of your life, or someone close to you lives with it, this is a reason to know that gentler mind-body practices are not a soft option: in these pooled trials they sat alongside resistance training at the top for pain relief and also improved walking capacity. It is equally worth knowing that stacking aerobic and resistance work together showed no clear advantage on walking here. The honest picture is that several forms of movement help, the differences between them are uncertain, and the format someone can sustain comfortably is a legitimate deciding factor.
