Why it matters
Dietary nitrate is one of the most heavily researched endurance supplements, with a long-standing claim that it improves performance in endurance sports. But the literature is a scatter of small trials across different sports, doses, fitness levels and test formats, and individual studies have pointed in different directions. Pooling them is the only way to estimate the average effect and, just as importantly, to see which kinds of performance actually improve. This review set out to answer that question in healthy adults doing endurance-based activity.
What they did
The authors searched five major databases without language restrictions for studies that compared nitrate supplementation with a placebo in adults performing an endurance-based activity, and that reported either a performance measure or a physiological stand-in for one. Seventy-three studies covering 1061 participants met the criteria. Risk of bias was assessed with the standard Cochrane tool, results were combined using a random-effects model, and the team rated their confidence in each pooled estimate using the GRADE approach.

Nitrate improved power output by an average of 4.6 watts, extended time to exhaustion by 25.3 seconds, and increased distance travelled by 163.7 metres. It also slightly reduced oxygen consumption during exercise, by 0.04 litres per minute, hinting at a small efficiency gain. Several outcomes did not move at all: perceived exertion, time-trial performance and total work done showed no significant difference, and neither did VO2max nor blood lactate. The overall pattern is a real but modest benefit that appears in open-ended tests more than in race-style ones.
Where it fits
The analysis supports the broad idea, long argued in sports nutrition, that nitrate supplementation benefits endurance performance. The interesting wrinkle is where the benefit lands: time to exhaustion improved while time-trial performance did not, and time trials are the format closest to competition. That split leaves open questions about whether the gains transfer to racing, and whether training status, dose or the specific sport changes the answer. The authors' own conclusion is suitably measured, describing benefits for performance-related outcomes rather than for races won.
What it means for you
If you are weighing up nitrate as an endurance aid, this is a reason to expect a small edge in how long you can sustain a hard effort, not a transformed race result. The headline gains, a few watts and under half a minute to exhaustion, are honest but modest, and the efficiency change was tiny. For recreational athletes the practical difference may be hard to feel. These were healthy adults on average, so any one person's response could sit above or below the pooled figure.
