Why it matters

Sleep problems are widespread: a 2021 World Health Organization survey found 27% of the global population experiences them, and middle-aged and older adults are more likely to have sleep disorders. Poor sleep is not just unpleasant, since sleep deprivation increases cardiovascular disease risk. Exercise is a common non-drug recommendation for better sleep, but the practical question has always been which kind. This study set out to rank aerobic exercise, resistance exercise, combined training and yoga against each other rather than simply asking whether exercise helps.

What they did

The researchers conducted a systematic review and network meta-analysis of randomised controlled trials comparing aerobic exercise, resistance exercise, combined training and yoga with control groups in middle-aged and older adults. Sleep was measured with the Pittsburgh Sleep Quality Index and its component scores, covering things like how long it takes to fall asleep, sleep duration, sleep efficiency and daytime function. Twenty-eight studies involving 3,460 participants were included, and the exercise types were ranked using cumulative ranking curves that estimate each one's probability of being best.

What they found

Aerobic exercise came out on top for the headline outcome, ranking most effective for total sleep quality score with a ranking value of 93.2%, and also led for sleep latency at 96.8% and for reduced sleep medication use at 77.1%. Yoga told a different story on the components: it ranked best for sleep disorders at 90.4%, sleep efficiency at 95.9%, sleep duration at 93.8% and daytime dysfunction at 98.3%. In other words, no single modality swept every measure; the two front-runners excelled at different aspects of sleep.

Where it fits

This extends a large body of evidence that exercise improves sleep by moving from whether to which. It also complicates the simple answer, because the best choice depends on which part of sleep someone wants to fix: overall quality and falling asleep faster favoured aerobic work, while efficiency, duration and daytime function favoured yoga. The rankings rest partly on indirect comparisons rather than trials that pitted the modalities directly against each other, which leaves open questions about optimal doses, intensities and combinations.

What it means for you

For a midlife or older reader sleeping badly, this is a reason to think regular movement is a genuine lever on sleep, and that the type of movement may shape which part of sleep improves. The pooled evidence puts aerobic exercise in the strongest position for overall sleep quality, with yoga a serious contender for specific components. It does not crown a single winner for everyone, and it says nothing about younger adults.
