Why it matters

Exercise programmes often produce smaller weight loss than the energy they burn would predict, and one popular explanation is compensation: the idea that a prescribed workout leads people to move less for the rest of the day, so total daily energy expenditure barely changes. If that were true, exercise-induced weight loss would be minimal or absent no matter how diligently someone trained. Whether healthy adults actually behave this way has real consequences for how exercise is prescribed for weight management. This review set out to weigh the evidence for the compensation hypothesis directly.

What they did

The authors searched PubMed and Embase for papers published between January 1990 and March 2013 that reported the change in non-exercise physical activity or energy expenditure in response to prescribed physical activity or exercise training in healthy adults. Eligible designs included cross-sectional studies, short-term studies lasting 2 to 14 days, and both randomised and non-randomised trials. Thirty-one articles met the criteria and were included. The question in every case was whether movement and energy expenditure outside the prescribed sessions fell once training began.

What they found

The great majority of the evidence pointed the same way. All four cross-sectional studies, 90% of the ten short-term studies and all seven randomised trials reported no reduction in non-exercise activity or energy expenditure in response to prescribed exercise. Non-randomised trials were more mixed, with half of the ten reporting no reduction. Overall, the authors found minimal evidence to support the idea that prescribed exercise makes healthy adults compensate by moving less at other times.

Where it fits

The compensation hypothesis has been widely used to explain why exercise trials often deliver disappointing weight loss, so a review finding little support for it complicates that narrative. The authors are careful about the limits: few included studies were adequately powered or designed specifically to test this question, and few measured both the exercise itself and everything outside it using the best available techniques, indirect calorimetry and doubly labelled water. That leaves room for small compensation effects to have gone undetected. Purpose-built trials with those measurements would settle the question more firmly.

What it means for you

If you have heard that starting a training programme quietly backfires because your body makes you slump on the sofa afterwards, this review is a reason to doubt that story, at least for healthy adults. The evidence suggests day-to-day movement generally holds up when structured exercise is added, so the energy a session burns is more likely to count towards your total than to be traded away. It also suggests that when exercise fails to shift weight, the explanation may lie elsewhere rather than in reduced background activity.
