Why it matters

Omega-3 fatty acids are among the most widely taken supplements for heart and metabolic health, yet the trial literature on whether they actually help people with metabolic syndrome and related cardiovascular disease has been openly inconsistent. Metabolic syndrome is a cluster of risk factors, so a supplement worth taking would need to show effects across blood fats, blood pressure and inflammation rather than on a single number. What they did

The researchers systematically searched PubMed, Embase and the Cochrane Library for randomised controlled trials up to 1 November 2022. Forty-eight trials involving 8,489 participants with metabolic syndrome and related cardiovascular diseases met the inclusion criteria. Results were combined as weighted mean differences using a random-effects model, with standard checks for publication bias and sensitivity. The team also ran subgroup analyses by dose and by patient group, and a meta-regression to test whether longer supplementation produced larger changes.

What they found

Omega-3 supplementation significantly reduced triglycerides by 18.18 mg/dl on average, total cholesterol by 3.38 mg/dl, systolic blood pressure by 3.52 mmHg and diastolic blood pressure by 1.70 mmHg, and it nudged HDL cholesterol up by 0.99 mg/dl. Several inflammatory markers, including IL-6, TNF-α, C-reactive protein and IL-1, also fell significantly. LDL cholesterol did not change, and neither did several markers of blood vessel activation. Subgroups suggested doses of 2 g per day or less were more beneficial overall, anti-inflammatory effects were stronger in heart failure patients, and longer supplementation was linked to bigger changes in triglycerides and inflammation.

Where it fits

This pooling helps reconcile a genuinely contradictory literature by showing where omega-3s reliably act and where they do not. The triglyceride effect is the standout and consistent with what many clinicians already expected, while the null result on LDL clarifies that these supplements are not a cholesterol drug substitute. The subgroup patterns, with different benefits landing in different patient groups, help explain why individual trials disagreed. What remains open is whether these marker improvements translate into fewer heart attacks and deaths, which this analysis of intermediate outcomes cannot answer.

What it means for you

If you think of fish oil as a general heart cure, this is a corrective in both directions. In people with metabolic syndrome or established heart conditions, omega-3s were linked to genuine improvements in triglycerides, blood pressure and inflammation, so the supplement is not doing nothing. But the average changes were modest, LDL cholesterol was untouched, and the trials studied patients rather than healthy adults, so the findings describe a specific population. It is a reason to see omega-3s as one modest lever among many for metabolic health rather than a standalone fix.
