Why it matters
Sleep problems are extremely common in pregnancy and after birth, and they feed into the overall well-being of both mother and child. Yet most sleep advice is written for the general adult, not for someone whose body and schedule are being reshaped by a pregnancy or a newborn. This review set out to consolidate the trial evidence and answer whether exercise actually improves sleep in perinatal women, rather than assuming benefits found in other groups carry over.

The authors systematically searched eight databases, including CINAHL, Cochrane, EMBASE, PsycINFO, PubMed, Scopus, Web of Science and ProQuest, for English-language studies from each database's inception to 7 December 2019. From 2,139 records identified, ten randomised controlled trials met the criteria, and seven of them supplied data that could be pooled. Together the trials involved 998 pregnant and postpartum women across eight countries. Study quality was graded with standard risk of bias tools, and the pooled results were evaluated for the size and consistency of the effects.
What they found
The results favoured exercise across the board. Women assigned to exercise interventions saw improvements in sleep quality, sleep efficiency, sleep duration and sleep latency, meaning they fell asleep faster and slept longer and better. Insomnia, diurnal tiredness and daytime dysfunction also improved, so the benefits were not confined to the night itself. The effects ranged from medium to very large, which for a free and low-risk intervention is a striking result. No sleep outcome in the pooled analysis went the other way.
Where it fits
This review consolidates a scattered set of trials into a coherent picture, and it strengthens the case for advocating exercise among perinatal women for sleep and general health. It does not settle which kinds of exercise, at what intensity or frequency, work best, and the pool of trials remains small. The authors also note that future trials should follow established reporting standards so the next generation of evidence is easier to trust and compare. For now, the direction of the finding is clear even if the fine print is not.

For anyone pregnant or recently postpartum and struggling with sleep, this is a reason to see movement as more than general health advice: the pooled trials point to genuine improvements in how fast women fell asleep, how long they slept and how they felt during the day.
