Why it matters

Older adults with mild cognitive impairment or subjective cognitive decline carry a greater risk of dementia, and that risk falls hardest on people from minority ethnic and socioeconomically disadvantaged backgrounds. Psychosocial and lifestyle interventions that target modifiable risk factors are one of the few hopeful routes for reducing that risk. Yet a programme only works if people actually change what they eat and how they move, and the behavioural mechanisms behind successful change have remained unclear. This review set out to identify which ingredients of an intervention make behaviour change happen in this group.

What they did

The authors searched PubMed, Embase, PsycINFO, Web of Science and Scopus for randomised trials testing interventions designed to improve lifestyle and cognition in older adults with memory concerns. They found 26 studies describing 23 trials, of which 14 were multidomain programmes and 9 targeted a single domain. Behavioural mechanisms were mapped using the COM-B model, which frames behaviour as the product of capability, opportunity and motivation. Effectiveness was synthesised overall and within underserved groups, with lower risk of bias studies given priority.

What they found

Moderate-certainty evidence showed that programmes providing a socially supportive environment and combining nutritional education with counselling for six or more months improved diet. Physical activity improved primarily where interventions included education, structured training and enablement strategies such as tailored programmes and providing tools. Interventions that targeted capability, opportunity and motivation at the same time were more effective overall. The certainty of evidence that behaviour change was associated with cognition was low. Only 13% of studies recorded ethnicity and 9% recorded sociodemographic status, and none explored their impact.

Where it fits

This review strengthens the case that non-pharmacological programmes can help people with memory concerns improve their lifestyles, provided they feel capable, equipped and motivated rather than simply informed. It also exposes a striking gap: the groups at greatest dementia risk, minority ethnic and disadvantaged populations, are barely represented in the evidence. The link between changing behaviour and actually improving cognition is still poorly established, which leaves the most important question in dementia prevention open. Future work will need to test tailored interventions in the populations most likely to benefit.
