Why it matters
Fish is the major natural food source of vitamin D, and dietary guidance often leans on that fact when recommending regular fish consumption. The authors searched Medline, Embase, Web of Science and the Cochrane Library up to February 2014 for randomised controlled trials comparing fish consumption against other dietary interventions and measuring blood 25-hydroxyvitamin D. Seven published articles plus 2 unpublished study data sets met the inclusion criteria, giving 640 participants across 14 study groups. The analysis separated fatty fish from lean fish and short-term studies lasting 4 to 8 weeks from longer studies of around 6 months. Mean differences in 25-hydroxyvitamin D concentration between fish and control groups were pooled.
What they found
Compared with controls, fish consumption raised 25-hydroxyvitamin D by an average of 4.4 nmol/L across 9 studies. The type of fish mattered a great deal: fatty fish produced a mean difference of 6.8 nmol/L across 7 study groups, with the studies agreeing closely, whereas lean fish produced only 1.9 nmol/L and that estimate was not statistically reliable. Duration also mattered. Short studies of 4 to 8 weeks showed 3.8 nmol/L, while the longer studies of around 6 months showed 8.3 nmol/L. The authors concluded that recommended fish intakes cannot optimise vitamin D status.
Where it fits
This confirms the textbook claim that fish supplies vitamin D while quietly deflating how much that supply is worth. The gap between fatty and lean fish fits what is known about where vitamin D sits in fish tissue, and gives a concrete reason to distinguish salmon or mackerel from cod. The larger effect in longer trials leaves open whether the ceiling here is genuine or simply a matter of time. It also leaves unanswered how fish compares against sunlight exposure or supplementation for someone starting from a low level.

This is a reason to treat fish as one contributor to vitamin D rather than a fix for a low reading. If you do eat fish for this purpose, the pooled evidence points clearly to oily varieties over white fish. The rises reported here are small in absolute terms, which is exactly why the authors say usual recommended intakes will not optimise status. Anyone concerned about their own level is looking at a question a blood test and a clinician answer, not a menu.
