Why it matters

What they did

The authors systematically searched PubMed, EMBASE, the Cochrane Library, Web of Science and Google Scholar up to October 2024 for randomised controlled trials in adult women with PCOS comparing GLP-1 receptor agonists against metformin or placebo. The primary outcomes were changes in body mass index, body weight, waist circumference, waist-to-hip ratio and abdominal girth. Secondary outcomes covered glucose handling, including fasting glucose, fasting insulin, oral glucose tolerance results and the HOMA-IR insulin resistance score, plus a panel of hormones, lipid profiles and safety.

What they found

GLP-1 receptor agonists significantly reduced BMI, body weight, waist circumference, waist-to-hip ratio and abdominal girth. On the metabolic side they lowered fasting insulin, blood glucose measured two hours after a glucose drink, and insulin resistance measured by HOMA-IR. Hormone levels, including total and free testosterone, DHEAS, SHBG and the free androgen index, did not change, and neither did most lipid measures; HDL cholesterol, usually considered the protective fraction, actually fell. The drugs also increased nausea, vomiting and dizziness compared with the comparators.

Where it fits

The weight and insulin findings echo what GLP-1 receptor agonists have shown in diabetes and general obesity populations, extending the picture to women with PCOS. The absence of any shift in androgens or other hormones is the more surprising element, since weight loss is often expected to improve the hormonal profile of the condition; it may mean the trials were too short, or that the drugs' benefits in PCOS run mainly through weight and insulin rather than hormones. The dip in HDL cholesterol is another loose thread. The authors call for longer studies of glucose and lipid outcomes to settle these questions.

For women with PCOS weighing up treatment options, this pooled evidence is a reason to think GLP-1 receptor agonists can deliver meaningful reductions in weight and insulin resistance where metformin and placebo fall short.
