Why it matters

Almost every weight-loss diet forces a trade-off: as the kilos come off, some of the loss is lean tissue, and resting metabolism tends to slow. Nutrition researchers have long argued over whether shifting the ratio of carbohydrate to protein, while keeping fat and total calories fixed, can tilt that trade-off in the dieter's favour. Individual trials pointed in different directions, which is why the question remained genuinely unsettled. This analysis set out to pool every trial that made a fair, calorie-matched comparison.

What they did

The researchers systematically searched MEDLINE, EMBASE, PubMed and the Cochrane register for weight-loss trials in adults aged 18 or over. Eligible trials prescribed the same calories and the same fat intake to both groups, differing only in how much protein replaced carbohydrate. Twenty-four trials covering 1063 people met the criteria, with an average diet length of 12.1 weeks. Pooled outcomes included body weight, fat mass, fat-free mass, resting energy expenditure, satiety and appetite, and cardiometabolic risk markers such as blood fats, glucose and blood pressure.

What they found

Higher-protein diets produced an extra 0.79 kg of weight loss and 0.87 kg of fat loss compared with standard-protein versions, plus a 0.23 mmol/L larger drop in triglycerides. They also softened the downsides of dieting, preserving 0.43 kg more fat-free mass and 595.5 kJ per day more resting energy expenditure. Fasting glucose, insulin, blood pressure and cholesterol changed similarly on both diets. Where satiety was measured, 3 of 5 studies reported people feeling fuller on the higher-protein plan.

Where it fits

This pooled analysis largely settles a question that single trials had answered inconsistently: when calories and fat are matched, extra protein helps, but modestly. The finding fits the broader picture that total energy intake drives most of the weight change, with macronutrient ratios fine-tuning what kind of tissue is lost. Because the average trial ran roughly 12 weeks, it cannot say whether the advantage grows, shrinks or disappears over years of real-world eating. Adherence outside a trial setting also remains an open question.

What it means for you

If you are eating in a calorie deficit, this is a reason to think the amount lost is set mainly by the deficit itself, while protein shapes the quality of the loss. The extra weight and fat loss from a higher-protein approach is real but under a kilogram on average. The stronger argument for protein here is protecting lean tissue and resting metabolism, and possibly feeling fuller along the way. None of this changes the fact that both diet styles worked when calories were controlled.
