But ageing is not a single-lever problem, and the practical question for anyone past midlife is which habits genuinely move outcomes like cognition, frailty and wellbeing — and whether combining them adds up to more than any single change. This review set out to evaluate multidimensional lifestyle interventions across nutrition, physical activity, mental health, social connection, harm avoidance, sleep and preventive healthcare. That breadth is its point: it asks where the evidence is strongest across the whole lifestyle picture, not within one silo.

What they did

The authors ran a systematic review and meta-analysis of randomised controlled trials published from 2014 through 2024, searching with MeSH terms including 'healthy aging', 'nutrition', 'physical activity', 'mental health', 'social connections' and 'preventive healthcare'. Studies qualified if they tested at least one lifestyle intervention in populations aged 50 years and above and reported a health or ageing-related outcome. What they found

Nutrition interventions — Mediterranean diet, antioxidant-rich foods and protein intake — were associated with cardiovascular benefits (RR 0.78), reduced cognitive decline (OR 0.72) and improved muscle mass (SMD 0.45). Aerobic and resistance exercise improved VO2 max by 3.6 mL/kg/min and lowered frailty risk (RR 0.67). Cognitive stimulation and mindfulness reduced dementia risk (OR 0.75) and stress (SMD -0.65), while social-support interventions cut susceptibility to depression by 30% (RR 0.70) and improved life satisfaction (SMD 0.55). Sleep of 7-8 hours was related to a 25% lower risk of cognitive decline (RR 0.75), CBT for insomnia strongly improved sleep quality (SMD 0.74), routine check-ups reduced undiagnosed chronic conditions by 40% (RR 0.60), and vaccination programmes cut influenza-related hospital admissions (OR 0.58).

Where it fits

The direction of these findings agrees with the broader healthy-ageing literature: diet, exercise, cognitive and social engagement, sleep and preventive care each carry evidence in their own right. What this review adds is a single synthesis arguing the domains are complementary rather than interchangeable. It also has clear limits: pooling 35 very different trials into domain-level estimates blurs what exactly was tested, and some entries — such as an association between moderate alcohol use and better liver function (SMD -0.38) — sit awkwardly and deserve sceptical reading. Which combinations matter most, at what intensity and for how long, remains open.

For readers past 50 — or planning to be — this is a reason to think the unglamorous basics are the ones with evidence behind them: a Mediterranean-leaning diet with adequate protein, a mix of aerobic and resistance training, staying cognitively and socially engaged, sleeping well and not skipping routine health checks.
