Why it matters
Leisure-time exercise is consistently linked to longer life, yet physically demanding jobs have repeatedly failed to show the same benefit and sometimes appear to do the opposite. One proposed explanation is the 'fit for work' principle: that heavy occupational demands are only harmful when a worker's cardiorespiratory capacity is too low to meet them, so the same job strains one person and merely trains another. Testing that idea requires large cohorts with objectively measured fitness and long enough follow-up to count deaths. This analysis set out to see whether fitness modifies the relationship between occupational physical activity and mortality.
What they did
Individual participant data were pooled from five European cohort studies, covering 9922 men with a mean age of 46.8 years (standard deviation 6.7). Occupational physical activity was captured by self-report and categorised into three levels, while cardiorespiratory fitness was measured objectively and split into low, moderate and high tertiles. The team used a two-stage approach, running Cox regression models within each cohort and then pooling the estimates with a random effects model, stratified by fitness. Models were adjusted for age, body mass index, smoking status, leisure-time physical activity and educational level.
What they found
Over an average 25.6 years of follow-up, 55.7% of the men died, and 29.3% of those deaths were from cardiovascular causes. Among men in the low-fitness group, high occupational physical activity was associated with increased cardiovascular mortality (hazard ratio 1.27, 95% CI 1.04-1.55) and all-cause mortality (HR 1.22, 95% CI 1.07-1.40) compared with low occupational activity. In men with high fitness, the cardiovascular association was attenuated and no longer statistically clear (HR 1.08, 95% CI 0.79-1.48). The all-cause estimate in the high-fitness group remained elevated but with a confidence interval crossing one (HR 1.27, 95% CI 0.98-1.83).
Where it fits
The result supports the 'fit for work' principle for cardiovascular death specifically, offering a mechanism-free but plausible account of why physically heavy jobs have not delivered the mortality benefit that leisure exercise does. It complicates the picture for all-cause mortality, where high fitness did not clearly remove the excess risk. Because occupational activity was self-reported and the design is observational, reverse causation and residual confounding cannot be excluded, even with adjustment for smoking, body mass index, education and leisure activity. The cohorts were all male and European, leaving open whether the same pattern holds in women or in other labour markets.
What it means for you
This is a reason to think that a physically demanding job does not substitute for being aerobically fit, and may sit differently on a body depending on its capacity. It also cautions against assuming that daily exertion at work covers your cardiovascular bases the way structured training does. The findings are associations drawn from decades-long observational cohorts of men, so no individual can read a personal risk off them. What they do suggest is that fitness and occupational workload are worth thinking about together rather than separately.
