The authors searched PubMed, Web of Science and Scopus for English-language human studies published between January 2000 and June 2026 that tested fasting interventions lasting at least 24 hours and reported thyroid hormone levels before and after. Fifteen studies qualified. Standardised mean changes were pooled using random effects models, keeping one estimate per study, corresponding to the longest eligible fast, to preserve statistical independence. Subgroup analyses compared fasts of three days or less against longer ones, sensitivity analyses tested different statistical assumptions, and meta-regression examined whether fasting duration tracked with the size of the hormonal change.

What they found

Fasting was associated with a pronounced drop in FT3, the active form of thyroid hormone, and a similarly clear fall in total T3. TSH, the brain's signal to the thyroid, showed a moderate decrease. FT4, by contrast, did not change significantly. If you practise extended fasting, this is a reason to expect T3 to read lower during or shortly after a long fast and to interpret that in context rather than as evidence of a broken thyroid. It is equally a reason not to over-extrapolate, since the studies involved fasts of at least 24 hours and shorter daily fasting windows are a different exposure entirely. Anyone with a diagnosed thyroid condition sits outside what these data describe, and nothing here says whether the hormonal shift itself is good, bad or neutral for long-term health.
