Why it matters
High-intensity interval training has become one of the most discussed exercise formats of the past two decades, promising big fitness returns from short sessions. Individual trials have suggested benefits across fitness, blood pressure, body composition and blood markers, but single studies are small and their results scattered. Guideline writers need large-scale analyses to know how big these effects really are and which markers actually move. This meta-analysis set out to provide exactly that, pooling the randomised evidence in the general population rather than in specific patient groups.

The researchers systematically searched PubMed, the Cochrane Library and Web of Science for randomised controlled trials published between 1990 and March 2023. Eligible trials tested a HIIT intervention against a non-intervention control group and reported at least one cardiometabolic health parameter. In total, 97 randomised controlled trials with a pooled sample of 3,399 participants were included, and weighted mean differences were calculated for each outcome across the pooled trials.
What they found
HIIT produced significant improvements in 14 clinically relevant cardiometabolic parameters. Peak aerobic capacity rose by 3.895 ml per minute per kilogram, systolic blood pressure fell by 3.203 mmHg, diastolic by 2.409 mmHg, and resting heart rate dropped by 3.902 beats per minute, alongside gains in left ventricular ejection fraction and stroke volume. Body composition improved too, with waist circumference down 2.843 cm and body fat down 0.972 percentage points. Fasting insulin, C-reactive protein, triglycerides and LDL cholesterol all fell while HDL cholesterol rose, though several of these blood-marker changes were small in absolute terms.
Where it fits
This is one of the largest pooled analyses of HIIT in the general population, and it strengthens the case that the format improves cardiometabolic risk factors broadly rather than just fitness. The authors suggest the findings may have implications for physical activity guideline recommendations. What it cannot say is how HIIT compares with moderate continuous exercise, since the comparison here was a non-intervention control, and it cannot identify which HIIT formats or durations work best. Those remain the live questions for future trials.
What it means for you
If you already do interval work, this is broad confirmation that it earns its place: against doing nothing, HIIT moved fitness, blood pressure, heart measures, body composition and blood markers together. The honest sizing is that each individual change was modest, so intervals are a contributor to cardiometabolic health rather than a shortcut to transforming it. It is also worth remembering the comparison was inactivity, so this study says nothing about whether HIIT beats steadier forms of cardio.
