Why it matters
Athletes and everyday exercisers regularly train and compete on poor sleep, whether from travel, nerves, work or life. Individual studies on how much this actually costs endurance performance have been small and scattered, leaving the practical question unanswered: how big is the hit, and does it depend on who you are, how much sleep you lost, or what kind of exercise you do? This review pooled every controlled trial comparing performance after sleep deprivation against performance after a habitual night's sleep in healthy people.
What they did
The authors searched four databases on 12 July 2022, plus reference lists and citations, for randomised and non-randomised controlled trials comparing sleep deprivation with a habitual sleep night in healthy humans. Thirty-one studies covering 478 people were analysed, generating 38 effect sizes in total, with study quality assessed using an established risk of bias tool. Subgroup analyses tested whether training status, the amount of sleep lost, assessment time, exercise mode and endpoint type changed the result.
What they found
Sleep deprivation had a moderate negative effect on endurance performance overall. The effect held regardless of training status, applying similarly to untrained, recreationally trained and trained people, and regardless of exercise mode across walking, running and cycling, endpoint type or assessment time. One or more nights of partial deprivation and one night of total deprivation compromised performance to a similar degree. The one factor that mattered was duration: exercise lasting more than 30 minutes was significantly more affected than shorter exercise. Only 8% of studies had low overall risk of bias, with 74% unclear and 18% high.
Where it fits
This puts a size on something widely assumed: poor sleep genuinely degrades endurance, and the effect is moderate rather than trivial or catastrophic. The duration finding is the most useful refinement, suggesting short, sharp efforts survive a bad night better than long ones. Two gaps stand out: the effect on well-trained and professional endurance athletes is unknown, and so is the impact of more than one night of total sleep loss. The generally unclear risk of bias across studies also tempers confidence in the precise size of the effect.
What it means for you
If you have a long run, ride or race after a bad night, this is a reason to expect a real but moderate performance cost, and the longer the effort, the more it is likely to show. Shorter sessions appear more resilient, which is useful context when deciding what a tired day is good for. The finding applied across training levels in these studies, so being fit does not appear to buy immunity, though elite athletes remain untested.
