Why it matters
Every rep has two phases: the concentric, where the muscle shortens as you lift, and the eccentric, where it lengthens under load as you lower. A long-running controversy asks whether these two dynamic muscle actions produce divergent hypertrophic responses, and therefore whether lifters chasing size should seek out ways to emphasise the lowering phase. This meta-analysis set out to answer that question directly, by pooling only trials where the two actions were compared with everything else held equal.
What they did
The authors systematically searched for randomised controlled trials in healthy adults that directly compared concentric and eccentric training without external implements and with all other training variables equivalent. Eligible studies had to run at least 6 weeks and measure changes in muscle size using biopsy, imaging such as MRI, CT or ultrasound, bioelectrical impedance or densitometry. Participants with injuries or health conditions expected to affect the growth response were excluded. Fifteen studies met the inclusion criteria and their results were pooled.
What they found
Eccentric training produced a larger effect on muscle growth than concentric training, but the difference did not reach statistical significance, meaning chance could not be ruled out. Across the studies, mean muscle growth was 10.0% with eccentric actions against 6.8% with concentric actions. Both modes clearly increased muscle size, and the authors conclude that both eccentric and concentric actions are effective, recommending that a hypertrophy-oriented resistance training programme include both.
Where it fits
The result sits between the two camps: it neither confirms a decisive eccentric advantage nor shows the two actions are identical. The consistent direction of the pooled effect keeps the eccentric hypothesis alive, while the lack of statistical significance means the case remains unproven on the available trials. The analysis was published in 2017 and restricted to healthy adults, so how the comparison plays out over longer training periods, in well-trained lifters, or across different muscles remains an open question.
What it means for you
Ordinary lifting already contains both phases, so most people are covered without changing anything. The pooled numbers are a reason not to invest in eccentric-only equipment or complicated negative-rep protocols on hypertrophy grounds alone, since the advantage over concentric work was not statistically secure. Equally, controlling the lowering phase rather than letting the weight drop costs nothing, and it keeps the potentially more effective half of every rep working for you.
