The reviewers searched four databases, PubMed, Embase, SportDiscus and PsycINFO, from inception to December 18, 2024, for randomised controlled trials of aerobic exercise in postmenopausal women. Sixty-one trials qualified, covering 4,100 women, of whom 2,356 were assigned to exercise and 1,744 to control conditions. Two reviewers independently selected studies, extracted data and assessed risk of bias, with a third resolving disagreements. Certainty of the evidence was formally graded, and outcomes covered blood pressure, the lipid profile, triglycerides, anxiety and depression.

Aerobic exercise reduced systolic blood pressure by 4.5 mm Hg and diastolic blood pressure by 2.4 mm Hg compared with control.
