The authors searched MEDLINE, Scopus and Web of Science up to October 2021 for randomised trials testing a synbiotic intervention against cardiometabolic and anthropometric outcomes in people with metabolic syndrome. Five trials with 1049 participants were included in the final analysis. Effects were pooled as weighted mean differences under a random effects model, with subgroup analyses to explore why studies differed and a dose-response analysis to test whether higher intakes produced larger effects.

Synbiotic supplementation reduced serum insulin by 6.39 μU/mL, triglycerides by 20.3 mg/dl, total cholesterol by 7.8 mg/dl, LDL cholesterol by 9.02 mg/dl, waist circumference by 4.04 cm, body weight by 4.3 kg and systolic blood pressure by 1.8 mmHg, while HDL cholesterol rose by 2.3 mg/dl and interleukin-6 fell slightly. Not everything moved. Fasting glucose, insulin resistance scores, BMI, diastolic blood pressure, heart rate and C-reactive protein were unchanged, and that mix of wins and flat results belongs in the same picture.

Where it fits

The findings strengthen the case that gut-targeted supplements can influence metabolic health, extending earlier probiotic research to combined formulations. Five trials is a modest foundation, and for several outcomes the studies disagreed considerably on the size of the effect, so the pooled numbers should be read as estimates rather than promises. The absence of change in fasting glucose and insulin resistance scores, despite lower insulin, also complicates the story. Larger trials would clarify which formulations, doses and durations drive the benefits.

What it means for you

For anyone whose measurements lean towards metabolic syndrome, this is evidence that synbiotics did more than nothing in trials: weight, waist, lipids and systolic blood pressure all improved on average. The 4.3 kg weight figure comes from only two of the five trials, so treat it as an early signal rather than an expectation.
