Why it matters
During the COVID-19 pandemic, huge numbers of people found themselves exercising in filtering respirators such as N95 and FFP2 masks, and many reported that they felt genuinely hard to breathe through. That raised a practical question with real stakes: does a tight-sealing respirator actually change what the heart and lungs are doing during aerobic exercise, or does it simply feel unpleasant? Individual trials had produced scattered answers, so this review set out to pool the randomised evidence on heart rate, breathing rate, blood oxygen and blood pressure in healthy people and settle the physiological side of the debate.
What they did
The authors searched the MEDLINE database for studies published in English between 2005 and 2021, and to reduce bias they accepted only randomised controlled trials and randomised crossover trials. The outcomes of interest were heart rate, respiratory rate, blood oxygen saturation and blood pressure, with perceived exertion assessed using the Borg scale, a standard rating of how hard exercise feels. Eight controlled trials met the criteria and were pooled, with different subsets of trials contributing data to each outcome.
What they found
None of the physiological measures shifted meaningfully when healthy people exercised in an N95 or FFP2 mask. Seven trials contributed heart rate data, five contributed respiratory rate, and five each contributed blood oxygen saturation and blood pressure; in every case the pooled result showed no significant effect. The exception was perceived exertion, which six trials assessed: the authors describe those results as controversial, pointing in different directions rather than agreeing. So the objective measures held steady while the subjective experience of masked exercise remained unresolved.
Where it fits
The review adds pooled weight to a question that individual trials had answered inconsistently, and its conclusion is reassuring: on the objective measures, respirator masks did not compromise healthy people during aerobic exercise. Because only randomised designs were included, the finding rests on the most reliable form of trial evidence available up to 2021. What it cannot answer is how masks affect people with heart or lung conditions, and whether the disagreement over perceived exertion reflects real differences between masks, exercise intensities or individuals. Those remain open questions for future trials.

If you have ever avoided a masked workout for fear of starving yourself of oxygen, this pooled evidence is a reason to relax: in healthy adults, heart rate, breathing rate, blood oxygen and blood pressure all held steady. The discomfort some people describe may well be real, since the trials could not agree on how hard masked exercise felt, but discomfort is not the same thing as physiological strain.
