Why it matters
Polycystic ovary syndrome is one of the most common endocrine disorders in women, characterised by excess body weight, elevated androgens, high blood sugar and insulin resistance, and it often brings hirsutism and fertility difficulties with it. Dietary strategies are known to ease the metabolic disturbances, hormonal imbalance and inflammation that accompany the condition. Intermittent fasting has attracted attention as a way to improve health outcomes and regulate circadian rhythm, but whether it specifically helps women with PCOS has remained unclear. This review set out to pool what trial evidence exists.
What they did
The authors conducted a systematic review and meta-analysis, searching PubMed, Scopus and Web of Science with no date restrictions for studies of intermittent fasting in women diagnosed with PCOS. Five studies met the inclusion criteria. The pooled analysis, run in Cochrane Review Manager, examined anthropometric measures such as body weight, BMI and waist-to-hip ratio; metabolic markers including fasting glucose, fasting insulin, insulin resistance and blood lipids; hormones including testosterone, DHEA-S, the free androgen index, SHBG and anti-Mullerian hormone; and the inflammatory marker C-reactive protein.
What they found
Intermittent fasting significantly reduced body weight by 4.25 kg and BMI by 2.05 kg/m2 on average. Metabolically, fasting blood glucose fell by 2.86 mg/dL, fasting insulin by 3.17 μU/mL, the HOMA-IR insulin resistance index by 0.94 and triglycerides by 40.71 mg/dL. On the hormonal side, DHEA-S dropped by 33.21 μg/dL, the free androgen index fell by 1.61% and sex hormone-binding globulin rose, while C-reactive protein, a marker of inflammation, fell by 2.00 mg/L. Not everything moved: waist-to-hip ratio, total cholesterol, LDL, HDL, total testosterone and anti-Mullerian hormone showed no significant change.
Where it fits
Dietary intervention is already a recognised route to easing PCOS's metabolic disturbances, and this pooled analysis suggests intermittent fasting belongs on the list of candidate approaches, with benefits reaching beyond weight into insulin resistance, androgens and inflammation. But the evidence base is thin: five studies is a small pool, and the authors explicitly describe the findings as preliminary and in need of more robust trials. The null results on total testosterone, cholesterol fractions and anti-Mullerian hormone also show the effects are selective rather than sweeping, leaving open exactly which aspects of PCOS fasting can and cannot shift.
What it means for you
For women with PCOS weighing up dietary approaches, this is a reason to see intermittent fasting as a genuinely promising option under study: the pooled trial data point to meaningful average improvements in weight, insulin resistance, triglycerides, free androgens and inflammation. It is equally a reason for measured expectations: total testosterone and cholesterol did not improve, and the whole picture rests on just five studies.
