Why it matters
Intermittent fasting regimens, most famously 5:2, are often sold as metabolically superior to ordinary calorie counting: same total restriction, better results. If true, that would change how millions approach weight loss; if false, people could stop treating fasting days as a magic ingredient. Individual trials have been small and mixed, so a systematic pooling of the randomised evidence is the fairest test. This review compared intermittent energy restriction directly against continuous restriction on weight, body composition, blood pressure and other cardiometabolic risk factors.
What they did
The researchers systematically searched MEDLINE, the Cochrane Library, TRIP, EMBASE and CINAHL up to May 2018 for randomised controlled trials pitting intermittent against continuous energy restriction. Eleven trials qualified, with durations ranging from 8 to 24 weeks. All the intermittent regimens provided no more than 25% of daily energy needs on fasting days, but varied in structure, some following 5:2 and others different patterns, and in whether eating on non-fasting days was ad libitum or balanced. Effects were expressed as weighted mean differences between the two approaches.
What they found
Weight loss was statistically indistinguishable between the approaches: the difference was 0.61 kg, and 0.38% in percentage terms, neither significant. The one measurable difference was a slight reduction in fasting insulin with intermittent regimens, 0.89 µU/mL, which the authors flag as of uncertain clinical relevance. Across the other outcomes examined, including body composition, blood pressure and other cardiometabolic risk factors, no between-group differences were found. Both approaches worked; neither worked better.
Where it fits
This pooling lands squarely on the side of energy balance: when total restriction is matched, the eating pattern used to achieve it made no detectable difference to weight over these timeframes. It complicates popular claims that fasting confers unique fat-loss advantages, while leaving room for the possibility that longer trials could reveal differences, something the authors explicitly call for. The small insulin finding keeps a metabolic question alive without answering it. Adherence over years, rather than weeks, remains the biggest untested variable.

If you have wondered whether you are leaving results on the table by not fasting, this is a reason to relax: over 8 to 24 weeks, the pooled trials found the pattern of restriction did not matter for the scale. Equally, if intermittent fasting makes restriction easier for you to sustain, nothing here says it works worse. The honest reading is that these are two doors into the same room.
