Why it matters

Exercise is a cornerstone of type 2 diabetes management alongside medication, but the prescription details are where the argument lives. Aerobic work has traditionally been the default recommendation, while resistance training has been added more cautiously, and it has not always been clear whether combining the two buys anything extra for glycaemic control. Body composition adds another layer, since changes in weight and fitness may travel alongside changes in blood glucose. This review set out to quantify the effect of exercise on glucose metabolism and body composition, and to test whether the modality makes a difference.

What they did

The authors searched Web of Science, PubMed, EMBASE and the Cochrane Library for literature up to 8 May 2026, and pooled the results using RevMan 5.4 with the Cochrane Risk of Bias Assessment Tool applied to study quality. Sixteen randomised controlled trials involving 1401 participants with type 2 diabetes were included. Outcomes covered glycated haemoglobin, fasting blood glucose and an insulin resistance index on the metabolic side, plus body mass index and maximal oxygen uptake for body composition and fitness. A subgroup analysis separated combined aerobic-plus-resistance programmes from aerobic exercise alone.

What they found

Exercise significantly reduced glycated haemoglobin overall (SMD -0.46, 95% CI -0.73 to -0.19). In the subgroup analysis, combined exercise produced the larger improvement (SMD -0.71, 95% CI -0.95 to -0.47) compared with aerobic exercise alone (SMD -0.32, 95% CI -0.62 to 0.02). Fasting blood glucose also fell significantly (SMD -0.52, 95% CI -0.70 to -0.35), and maximal oxygen uptake rose (SMD 0.58, 95% CI 0.34 to 0.82). Two results were less clean: the insulin resistance index showed no statistically significant improvement (SMD -0.18, P = 0.11), and the reduction in body mass index reached significance only after an outlier study was excluded (SMD -0.37).

Where it fits

The finding reinforces the now-common recommendation to combine cardio with resistance work rather than treating aerobic training as sufficient on its own, and puts a pooled number on the gap between the two approaches. The lack of significant movement in the insulin resistance index is a genuine complication, since it suggests the glycaemic improvements are not fully captured by that particular marker in these trials. The BMI result being contingent on removing an outlier is a reminder that body composition effects in exercise trials are often fragile. Open questions include how long the benefits persist and which specific programme variables drive the difference.

What it means for you

If you are managing blood glucose and treating cardio as the whole job, this pooled evidence is a reason to view resistance work as part of the same picture rather than an optional extra. The size of the effect is moderate, not transformative, and exercise here sits as a non-pharmacological component of management rather than a replacement for anything. Notably, the benefits to HbA1c and fasting glucose showed up without a dependable reduction in body mass index, which suggests weight change is not the only route to better numbers. Fitness itself improved, which matters independently.
