What they did

The authors searched Medline, the Cochrane Library, Web of Science and Google Scholar for trials published up to April 2019, turning up 2327 potentially relevant studies. Fifteen randomised controlled trials met the inclusion criteria. The outcomes of interest were stool frequency, whole-gut transit time, stool consistency and bloating. Two authors independently handled study selection, risk-of-bias assessment and data extraction, and the results were combined using a model that allows for differences between trials.

Overall, probiotics shortened whole-gut transit time by 13.75 hours and increased stool frequency by 0.98 bowel movements per week. The more interesting pattern sat underneath: products containing at least two bacterial species drove the benefit, adding 1.22 movements per week, while Bifidobacterium lactis and Bifidobacterium longum taken alone showed no significant effect on frequency. The same split appeared for stool consistency and bloating, where multi-species products improved both and single-species products moved neither significantly. Reporting quality was a weak point, with a high risk of bias in how the trials were run.

Where it fits

This pooled analysis pushes a muddled literature towards a clearer answer: probiotics can help functional constipation, but the formulation appears to matter a great deal. The consistent advantage of multi-species blends over single strains is the finding most likely to change how future trials are designed and how products are compared. What it cannot say is which specific combinations work best, at what doses, or for how long benefits last. And because many trials were poorly reported, the true effects may be smaller than the pooled numbers suggest.

If constipation is a recurring problem, this is evidence that probiotics are more than marketing, with measurable effects on transit time and weekly frequency.
