Why it matters

Sarcopenia is a skeletal muscle disease marked by low muscle strength, reduced muscle quantity or quality, and declining physical performance, and it erodes independence in later life. The authors registered a protocol in advance and systematically searched the MEDLINE and CINAHL databases for randomised controlled trials published between 2013 and 2023. Eligible studies tested nutrition-only interventions and measured muscle mass, muscle strength or physical function in female participants who were middle-aged or older. Fourteen trials met the selection criteria. The included studies varied considerably in the type and amount of supplementation, the age of participants and how long the interventions ran.

Six of the fourteen trials reported benefits: protein, vitamin D, vitamin D combined with magnesium, and fish oil each improved muscle protein synthesis, muscle strength or muscle function in at least one study. The other eight trials, covering various protein interventions, vitamin D alone, magnesium alone and dairy derivatives, showed no effect. Exercise appeared to modulate the results in several of the studies. Taken together, the picture is of a limited but positive effect from nutrition alone.
