Why it matters
Omega-3 polyunsaturated fatty acids have been shown to decrease the risk of several components of the metabolic syndrome, which naturally raised the question of whether they also improve how the body handles glucose. Insulin sensitivity sits at the centre of metabolic health, and a cheap, widely used supplement that improved it would matter to a lot of people. Yet the role of omega-3 in glucose metabolism was unclear, with the evidence scattered across small trials. The authors followed Cochrane guidelines for systematic reviews and searched MEDLINE, EMBASE, CENTRAL and clinicaltrials.gov from the beginning of each database until October 2010. From 303 screened citations, 11 randomised controlled trials met the inclusion criteria, together enrolling 618 participants. Results were pooled using a random effects model to estimate a standardised mean difference between omega-3 and placebo. The team also ran subgroup analyses, comparing people with type 2 diabetes against other populations and comparing trials by the method used to measure insulin sensitivity.
What they found
Pooled across all 11 trials, omega-3 supplementation had no effect on insulin sensitivity compared with placebo, with a standardised mean difference of 0.08. The subgroup analyses largely told the same story: no effect in people with type 2 diabetes, no effect in other populations, and no effect in trials using the QUICKI measure or other tests. The one exception was the subgroup of trials using the HOMA measure, where omega-3 was associated with a statistically significant increase in insulin sensitivity of 0.30. The authors conclude the overall picture is consistent with a lack of effect.
Where it fits
This pooled null complicates a tidy narrative in which omega-3, already linked to lower risk of several metabolic syndrome components, should also improve glucose handling. The isolated positive finding in the HOMA subgroup is hard to interpret when every other way of measuring insulin sensitivity showed nothing, and it may reflect the measurement method rather than a real biological effect. The search closed in October 2010, so trials published since are not included, and whether newer or larger studies would shift this picture remains an open question.
What it means for you
If omega-3 is part of your routine, this is a reason to keep your expectations specific: whatever else it may do, the pooled trial evidence here does not support taking it to improve insulin sensitivity. The single positive subgroup result is thin ground next to a null across every other analysis. For anyone thinking about metabolic health, this points attention back towards approaches with stronger evidence behind them rather than towards the fish oil bottle. It says nothing about omega-3's effects on other outcomes.
