The researchers searched five databases, including PubMed, Embase and Web of Science, up to 31 October 2025 for randomised controlled trials lasting at least four weeks that compared structured aerobic exercise with inactive or minimal control conditions. Seventeen trials involving 2,865 women met the criteria. Depressive symptoms, anxiety and health related quality of life were measured with validated instruments, and the team used model based methods to trace how benefits changed with weekly exercise volume, expressed in MET minutes.

What they found

Aerobic exercise significantly reduced depressive symptoms, with larger improvements in women who were depressed at baseline and in programmes delivered postpartum. The dose response curve was U shaped, with the greatest benefit at approximately 570 MET minutes per week. Quality of life also improved, following an inverted U shape that peaked near 420 MET minutes per week. Anxiety was the exception: no significant effect emerged, so the evidence cannot yet support aerobic exercise as an anxiety remedy in this group.
