Why it matters
Population studies have long suggested that physically active people keep their cognitive faculties for longer, but that kind of evidence cannot separate cause from effect: perhaps sharper people simply move more. The strongest test is a randomised trial, and the people with most to gain are arguably those who currently move least. Yet trial evidence in specifically sedentary older adults has been limited and inconsistent. This review set out to pool what randomised trials exist and ask whether getting sedentary older people active genuinely improves their cognition.

The authors searched six international databases, including PubMed, the Cochrane Library and Web of Science, plus three Chinese databases, for eligible studies published up to January 1, 2021. Seven randomised controlled trials met the criteria, comprising 321 sedentary older adults, with 164 assigned to physical activity and 157 to control conditions. Effects on cognition were estimated using standardised mean differences under a random-effects model, publication bias was checked with funnel plots, and subgroup analyses examined baseline cognition, intervention duration, activity type and country.

Overall, physical activity significantly improved cognition compared with control conditions, with a moderate pooled effect. The subgroup analyses pointed to where the benefit concentrated: participants with cognitive impairment at baseline, interventions lasting longer than 12 weeks, aerobic forms of exercise, and trials conducted outside the United States all showed significant effects.
