Prediabetes converts to type 2 diabetes at a considerable rate if nothing changes, which makes it one of the highest-value windows for diet-based intervention. Prebiotics — food components that feed gut bacteria — have been promoted as a way to improve glycaemic control through the microbiome, and individual trials have been run in exactly this population. The results, however, have been contradictory, with some trials reporting metabolic benefit and others none. This review set out to establish whether prebiotic consumption favourably alters metabolic status and body composition in adults with prediabetes.

What they did

The authors searched electronic databases up to January 2024 for randomised clinical trials testing prebiotic consumption against a control in adults with prediabetes, with outcomes covering glycaemic status, lipid profile and anthropometric measures. Eleven studies met the criteria, contributing 546 participants in total: 258 allocated to prebiotic supplementation and 288 to control. Pooled outcomes included fasting plasma glucose, insulin, glycosylated haemoglobin A1c, HOMA-IR, total cholesterol, triglycerides, LDL and HDL cholesterol, plus body fat percentage, body mass index and waist circumference. The review was registered in advance on PROSPERO, and the certainty of each pooled estimate was formally graded.

What they found

Body fat percentage fell significantly with prebiotic supplementation, with a standardised mean difference of -1.27% and a 95% confidence interval running from -2.33 to -0.22. No significant change appeared in any of the metabolic indices: fasting plasma glucose, insulin, HbA1c, HOMA-IR, total cholesterol, triglycerides, or LDL and HDL cholesterol all held steady. The other anthropometric measures, body mass index and waist circumference, were likewise unaffected. The authors conclude there was no fair evidence that prebiotics improved metabolic or anthropometric features overall, and flag very low certainty in the pooled estimate for the majority of outcomes.

Where it fits

This complicates the popular story that feeding the gut microbiome reliably improves blood sugar control. Where the mechanistic case for prebiotics is strong, the pooled human evidence in the group with most to gain is thin and inconsistent. The body fat finding is intriguing, and the authors argue it could hint at improved insulin sensitivity, but it sits on a wide interval and stands alone among a set of null results. Whether longer trials, higher intakes or specific prebiotic types would produce metabolic change remains genuinely open.

What it means for you

This is a reason to keep expectations modest about prebiotic supplements as a way to move glucose numbers if you are in the prediabetes range. Nothing here says prebiotics are harmful, and the body fat signal may be worth watching, but the glycaemic markers people actually track did not shift in the pooled analysis. Certainty in most of those estimates is very low, so this is an absence of demonstrated benefit rather than proof of no benefit. It does suggest that a supplement is unlikely to do the work that broader dietary and activity changes are known to do.
