Why it matters
Probiotic supplements are sold largely on the promise that they tune the immune system, and the main mechanism proposed for them is immunomodulation: the idea that swallowed microorganisms shift circulating immune and inflammatory factors in ways that benefit the host. Whether that actually happens in healthy adults, as opposed to people with a disease, had never been evaluated in a systematic review before this one. The gap matters because healthy people make up a large share of the probiotic market, and they are often buying on the strength of a proposed mechanism rather than demonstrated outcomes in people like them.
What they did
The authors searched PubMed, SCOPUS, ISI Web of Science, ProQuest and Cochrane databases for English-language papers up to May 15, 2019, and checked the references of relevant papers for more. They included only randomised controlled trials comparing probiotic supplements with placebo on immune and inflammatory markers in healthy adults aged 18 to 65, without acute or chronic disease and in generally good health. Two authors extracted the data independently, and each study's quality was appraised using the Academy of Nutrition and Dietetics tools. Eighteen articles covering 819 subjects met the criteria; 13 were rated high in quality and five neutral.
What they found
Eight of the 18 articles reported a significant effect on immune or inflammatory parameters, including increases in natural killer cells, lymphocytes and monocytes, and decreases in proinflammatory cytokine concentrations. The majority, however, did not, and the overall conclusion was that probiotic supplementation has a limited effect on immune and inflammatory markers in healthy adults. The evidence was so mixed across studies that the authors could not pool it into a meta-analysis, and they found it difficult to aggregate the results or draw any conclusion about the size of an effect.
Where it fits
By the authors' account, this was the first systematic evaluation of the immunomodulation claim specifically in healthy adults without disease, and its verdict complicates a central selling point of the category. Individual positive trials clearly exist, but they sit within a body of evidence too inconsistent to support a general claim about immune benefits. Open questions remain about whether particular products, regimens or subgroups of healthy people respond differently, and whether findings from patient populations carry over to the healthy. Larger and more consistent trials would be needed to move the verdict in either direction.
What it means for you
If you are healthy and take a probiotic in the hope of measurably strengthening your immune system, this review is a reason to hold that expectation loosely. Some trials do show shifts in immune cells and inflammatory signals, but the overall picture in healthy adults is one of limited and inconsistent effects that could not be summed into a single estimate. Nothing here speaks to digestion or specific gut complaints; it addresses the blood-level immune claim, and on that claim the evidence in healthy people is thin.
