'Healthy obesity' is not risk-free, but fitness may explain the gap

Why it matters

The 'fat but fit' debate turns on whether obesity without metabolic problems, meaning normal blood pressure, blood sugar and lipids, is genuinely benign. If metabolically healthy obesity carries no extra risk, the scale matters far less than the blood panel; if it does, the reassurance is false. A second question sits underneath: are metabolically healthy obese people healthier because of something about their bodies, or because they simply move more and are fitter? Untangling activity and fitness from the obesity phenotype is exactly what this review set out to do.

What they did

The researchers systematically reviewed and meta-analysed two bodies of evidence. First, 67 studies comparing physical activity, sedentary behaviour, cardiorespiratory fitness and muscular strength between metabolically healthy and metabolically unhealthy obese people, taken at a single point in time. Second, 11 studies following people over the years to compare all-cause mortality and cardiovascular disease outcomes in metabolically healthy obese individuals against metabolically healthy normal-weight individuals, the best-case comparison group, after adjusting for activity, sitting time, fitness or strength. The outcome analysis drew on 10 unique cohorts, 100% of them scored as high quality.

What they found

Metabolically healthy obese people were more active, spent less time sedentary and had higher cardiorespiratory fitness than their metabolically unhealthy counterparts, with no difference in muscular strength. That pattern suggests their better metabolic profile may be at least partly down to lifestyle. Even so, after accounting for physical activity, they carried a 24-33% higher risk of all-cause mortality and cardiovascular disease than metabolically healthy normal-weight people, a difference described as borderline significant. Notably, that risk estimate was lower than in previous meta-analyses that had not accounted for activity, and the single study examining fitness suggested it largely explained the remaining gap.

Where it fits

This analysis complicates the comforting idea of harmless obesity: even the healthiest obese phenotype showed elevated risk compared with the best-case scenario. At the same time, it softens the picture painted by earlier meta-analyses, since accounting for physical activity appeared to modestly shrink the risk estimates. The biggest open question is fitness: with only one study examining cardiorespiratory fitness in relation to prognosis, the suggestion that fitness explains most of the residual risk is provocative but thinly supported. The strength finding, no difference between phenotypes, also invites more work on which attributes actually track with metabolic health.

What it means for you

For a reader carrying extra weight with reassuring blood work, this is a reason not to treat that reassurance as the whole story, since the pooled data still showed elevated risk against normal-weight peers. But it is equally a reason for optimism about what is modifiable: the healthy-obese group was distinguished by moving more, sitting less and being fitter, and the limited evidence available hints that fitness itself may carry much of the protection. The honest summary is that metabolic health, activity and fitness travel together, and fitness may be the lever.
