Why it matters

Self-monitoring, the habit of recording what you eat, what you weigh and how much you move, has long been a core component of behavioural weight loss treatment. As tracking has migrated from paper diaries to websites, apps, wearables and smart scales, three questions have gone largely unanswered. How is digital tracking actually being used in weight loss programmes, how well do people stick with it, and does more tracking actually go hand in hand with more weight lost? This review set out to answer all three across a decade of trials.

What they did

The authors searched six databases for randomised controlled trials of behavioural weight loss interventions in adults with overweight or obesity. To qualify, interventions had to last at least 12 weeks, report weight outcomes at 6 months or beyond, and report both self-monitoring engagement and its relationship to weight change. Thirty-nine studies published between 2009 and 2019 met the criteria, covering 67 interventions that used digital self-monitoring. The team catalogued what was tracked, through which technology, how engaged participants stayed, and how engagement related to weight loss.

What they found

Greater digital self-monitoring was linked to weight loss in 74% of the occurrences where the relationship was tested. Physical activity was tracked in 82% of interventions, diet in 81% and weight in 72%, with websites the most common tool, followed by mobile apps, wearables, electronic scales and text messaging. Engagement was the weak point: few interventions achieved tracking on at least 75% of days. Digital arms showed higher engagement than paper-based arms in 21 of 34 comparisons and lower in just 2, while adding counselling made little difference to engagement compared with standalone digital programmes.

Where it fits

The finding confirms in the digital era what behavioural research had long suggested on paper: self-monitoring and weight loss consistently travel together. Because the link is an association observed within trials, it cannot show whether tracking drives weight loss or whether people who are losing weight simply track more. The engagement problem is the clearest open question, since even purpose-built digital tools failed to keep most participants logging regularly. How to sustain tracking over months, rather than which gadget to choose, looks like the field's harder challenge.

What it means for you

If you are using an app, a wearable or a smart scale to manage your weight, this is a reason to think the logging habit itself is the ingredient most consistently tied to results, more than any particular technology. It is also a reason to expect that keeping the habit going will be the hard part, since most trial participants could not sustain near-daily tracking either. The evidence describes an association, so tracking is best understood as a tool that accompanies weight loss rather than a guaranteed cause of it.
