Why it matters

Osteoarthritis of the hands is a stubborn source of daily pain, stiffness and lost grip, and it affects the joints people use for almost everything. Heat is one of the oldest and cheapest home remedies for aching joints, applied through paraffin baths, warmed mud, heated gloves and similar methods. Yet despite how widely warmth is used, its actual clinical value for hand osteoarthritis had not been pinned down, and it was unclear whether the way heat is delivered changes the result. This review set out to answer both questions systematically.

What they did

The researchers ran a systematic review and meta-analysis of randomised controlled trials, searching databases up to 19 December 2025. From 13 identified studies, 9 trials covering 627 people met the bar for pooling. They assessed each trial's risk of bias, then calculated standardised differences between heat therapy and comparison groups for pain, hand function and grip strength. They also ran subgroup analyses comparing multimodal heat, meaning paraffin's warmth plus mechanical compression or mud's warmth plus chemical and mechanical stimulation, against pure thermal treatment such as heated gloves, and used meta-regression to test whether treatment duration and frequency mattered.

What they found

Heat therapy significantly reduced pain on the Visual Analog Scale, with a standardised difference of -0.60, and improved scores on a hand-specific pain index and a health questionnaire. Grip strength also improved, with a standardised difference of 0.43. Other outcome measures showed no significant change. In subgroups, multimodal therapy beat monotherapy for both pain, at -0.85, and grip strength, at 0.55. Meta-regression found that longer single-treatment sessions and higher weekly treatment frequency were key moderators of better grip strength recovery.

Where it fits

This pooling gives one of the clearer quantitative pictures yet of a remedy that has long been used on tradition more than trial data. It supports heat as a genuine short-term option for hand osteoarthritis, and it adds a practical wrinkle: warmth combined with compression or mineral stimulation appears to do more than warmth alone. The open questions are real, though. The trials differed in how heat was applied, only short-term outcomes were assessed, and it remains unknown whether repeated courses change the course of the disease itself.

What it means for you

If your hands ache from osteoarthritis, this is evidence that heat is more than a comfort ritual: pooled trial data show measurable short-term relief in pain and a real bump in grip strength. It is also a reason to think the delivery method matters, since paraffin and mud approaches that pair heat with pressure or mineral stimulation outperformed plain heated gloves in these trials. Session length and weekly frequency tracked with better grip recovery too. None of this speaks to long-term benefit, which the trials did not measure.
