Why it matters

Elevated glucose and insulin levels are major risk factors for cardiometabolic disease, and aerobic exercise is one of the most widely recommended tools for improving glycaemic control. Yet most of that recommendation rests on the effects of regular training over weeks and months. What a single session of cardio does to blood glucose, insulin and glucagon in the moment had never been systematically reviewed in healthy adults. That matters because the acute response is the building block of the long-term benefit, and because it could reveal whether the state you exercise in changes what you get from it.

What they did

The researchers searched eleven databases from inception to May 2020 for randomised crossover studies in healthy adults. To qualify, a study had to measure glucose, insulin or glucagon before and immediately after a single bout of continuous aerobic exercise of at least 30 minutes, compared against a time-matched resting condition. In total 42 papers covering 51 studies were included: 45 studies with 391 participants for glucose, 38 studies with 377 participants for insulin, and 5 studies with 47 participants for glucagon. The team then ran pooled analyses, plus subgroup analyses by fed or fasted state and exercise mode, and examined whether age, body composition, fitness, duration and intensity shifted the results.

What they found

Overall, a single bout of aerobic exercise had no significant effect on blood glucose, with a pooled difference of 0.05 mmol/L. Insulin fell significantly, by 18.07 pmol/L on average, and glucagon rose significantly, by 24.60 ng/L, regardless of whether people had eaten. The most interesting result came from splitting studies by metabolic state: when exercise followed a meal, glucose fell by 0.27 mmol/L and insulin by 42.63 pmol/L, whereas fasted exercise moved neither significantly. Glucose responses were also moderated by exercise duration and maximal aerobic capacity. The studies disagreed with each other substantially throughout, and the evidence was rated moderate quality.

Where it fits

This is the first systematic pooling of the acute glucose, insulin and glucagon response to continuous cardio in healthy adults, and it supports the standing advice that aerobic exercise helps glycaemic control. It adds a useful refinement: the immediate blood sugar benefit appears specific to the fed state, which chronic training studies could not show. Open questions remain. The wide disagreement between studies means the true effect sizes are uncertain, the glucagon result rests on very few participants, and it is unknown how these acute responses translate for people who already have impaired glucose control.

What it means for you

If you already do continuous cardio of half an hour or more, this is a reason to think the session does more for your immediate blood sugar handling when it comes after food rather than on an empty stomach. It is also a reason not to expect fasted cardio to deliver an acute glucose benefit, since in these pooled results it did not. None of this speaks to fat loss, fitness gains or long-term health, which were not measured here. The findings apply to healthy adults, so anyone managing a diagnosed condition is outside what this evidence covers.
