Why it matters
Long stretches spent sitting have become a defining feature of modern working life, and wearables are the most popular consumer tool for nudging people to move more. But a wearable can be deployed in different ways: on its own as a self-monitoring device, alongside online coaching and social media support, or combined with face-to-face seminars and courses. Which of those strategies actually reduces sitting in healthy adults has been unclear, because individual trials rarely compare them directly. A network meta-analysis can rank the strategies by combining direct and indirect comparisons across trials.
What they did
The reviewers searched seven databases up to 1 June 2024 for randomised controlled trials testing wearable-based interventions against sedentary time and prolonged sitting in healthy adults. Twelve trials with 2957 participants qualified. Risk of bias was assessed with Cochrane tools, and the analysis compared five strategies: wearable only, wearable plus online support, wearable plus offline contact such as seminars, traditional non-wearable interventions, and control conditions such as maintaining a daily routine. Results were expressed as standardised mean differences, and a probability ranking estimated which strategy was most likely to be best.
What they found
Wearables combined with online support reduced total sedentary time significantly more than control conditions, traditional non-wearable interventions and wearables used alone, with pooled effects in the moderate to large range. In the probability rankings, wearable plus online came top for sedentary time at 98.1%, ahead of wearable plus offline contact and the wearable-only approach. The picture for prolonged sitting was different: no strategy showed a significant advantage over any other, and the reviewers note that research on unbroken sitting specifically is scarce. Rankings for prolonged sitting followed a similar order but without clear separation.
Where it fits
The result fits a wider pattern in behaviour change research: a monitoring device supplies information, but the coaching, feedback and social support wrapped around it appear to drive the actual change. It also exposes a genuine gap, since total daily sedentary time and long unbroken bouts of sitting are different problems, and the pooled trials had little to say about breaking up the latter. Whether online support keeps working over the long term, and which of its ingredients matter most, are questions the included trials cannot answer.
What it means for you
If you bought a tracker hoping it would get you out of your chair, this is a reason to temper expectations of the device on its own: the wearable-only approach was significantly outperformed here. The combinations that worked paired the device with online coaching and social media support. It is also worth knowing that even the best strategy showed no significant effect on prolonged unbroken sitting, so a falling daily total does not necessarily mean shorter stretches at the desk. This describes strategies on average, not a verdict on any particular product.
