Why it matters
Wearables have moved from step counters to devices that monitor sleep in detail, and increasingly they are used to deliver sleep interventions rather than merely record the night. That raises a practical question: does a sleep programme delivered through a device on your wrist or body actually improve how people sleep? Individual trials have tested this in different populations with different devices, but the overall picture was unclear. This review set out to evaluate the effectiveness of wearable-delivered sleep interventions on adult sleep outcomes, and to explore which factors affect total sleep time.
What they did
The authors searched eight databases for relevant English-language studies from inception until 23 December 2021. Twenty randomised controlled trials qualified, involving 1608 adults across nine countries. Each trial compared a sleep intervention delivered through a wearable against a comparator. The risk of bias in every trial was assessed with the Cochrane tool, and the certainty of the overall evidence was graded with the standard GRADE criteria. Results across trials were then pooled to estimate effects on sleep outcomes including sleep disturbance, sleep-related impairment and overnight breathing measures.
What they found
Wearable-delivered interventions produced significant improvements in two breathing-related measures of sleep: the oxygen desaturation index improved by 1.96 events per hour and the respiratory distress index by 3.13 events per hour. The pooled analyses also found significant reductions in sleep disturbance and in sleep-related impairment compared with the comparator groups, with the impairment result showing a notably wide range across trials. The authors' overall judgement was measured: these interventions may complement usual care rather than replace it.
Where it fits
This is one of the first attempts to pool the trial evidence on wearables as a delivery channel for sleep interventions, rather than as passive trackers, and the direction of the findings is positive. It extends the sleep intervention literature into consumer-adjacent technology, but it does not close the case: the authors explicitly call for more rigorous randomised trials with long-term follow-up in a wider range of populations. How durable the benefits are, and which components of the interventions do the work, remain open questions.
What it means for you
If you own a wearable, this is a reason to think of it as more than a nightly report card: interventions delivered through such devices improved several sleep outcomes across 20 trials. The gains came from structured sleep interventions using the device, so the finding does not mean that simply strapping one on improves sleep. The evidence is positive but early, and the authors' own framing, a complement to usual care, is the honest reading. Anyone with a suspected sleep disorder still has reasons beyond a gadget to get it looked at properly.
