Why it matters
Musculoskeletal pain is one of the commonest reasons people lose working days, and neck, shoulder and back complaints are widespread among employees who spend long stretches in one posture. Employers have responded with workplace exercise schemes, from supervised sessions to structured activity breaks, on the assumption that movement reduces pain. Whether that assumption survives contact with pooled trial data, and whether it holds equally across body regions, is a separate question. This review set out to test how effective such programmes are and how large the effect really is.
What they did
The authors searched four databases, PubMed, EBSCO, Web of Science and Cochrane, for research trials published between January 1990 and March 2013. Eligible studies had to include a comparison group of employees and to assess a physical activity programme alongside musculoskeletal pain and health-related behaviours. Results were combined using meta-analysis, with effects expressed as standardised mean differences using Hedges' g. Findings were then broken down by body region, including general pain, neck and shoulder pain, low back pain, and pain in the arm, elbow, wrist, hand or finger.

Two outcomes reached significance. General musculoskeletal pain was lower in the intervention groups, with a standardised mean difference of -0.40, and neck and shoulder pain was also lower, at -0.37. Both effects are modest to moderate in size rather than dramatic. The studies covering low back pain and pain in the arm, elbow, wrist, hand or finger were too few to produce statistically significant evidence, so those regions were left unresolved rather than shown to be unresponsive. The authors called for more work specifically on those areas.
Where it fits
This supports the broad idea that activity at work reduces pain, but it narrows the claim considerably: the consistent evidence sits with general pain and with the neck and shoulders. Low back pain, which is the outcome employers most often invoke when funding these schemes, was not settled here for lack of trials. Because pain is self-reported and participants know which group they are in, some of the improvement may reflect expectation as well as physiology. Open questions include which type and dose of activity works, how long benefits last after a programme ends, and whether office and manual workers respond alike.
What it means for you
For desk-bound neck and shoulder discomfort, structured activity during the working day is one of the better-evidenced options on the table, with an effect that is real but moderate rather than transformative. If your main complaint is your lower back, this pooling does not yet speak to you in either direction. The trials tested organised workplace programmes rather than individual habits, so it is context for what employers offer as much as for what you do alone. Nothing here specifies a type of exercise or an amount.
