Why it matters

Whether exercise makes you sharper, not just fitter, is a question with obvious stakes for an ageing population. Earlier reviews had suggested aerobic training benefits the brain, but they were hampered by methodological shortcomings and had been overtaken by several large randomised controlled trials published in the intervening years. This meta-analysis set out to update the picture using only randomised trials, asking whether supervised aerobic exercise training genuinely improves attention, memory and executive function in adults.

What they did

The authors systematically searched the literature from January 1966 to July 2009 for randomised controlled trials in adults aged 18 or over. To qualify, a trial had to last longer than a month, involve supervised aerobic exercise training, include a non-aerobic-exercise control group and report enough information to derive effect sizes. Twenty-nine studies met the criteria, contributing data from 2,049 participants and 234 separate effect sizes spread across different cognitive domains.

What they found

Adults randomised to aerobic training showed modest but statistically significant improvements across three domains: attention and processing speed at an effect of 0.158, executive function at 0.123 and memory at 0.128. All three effects are small in conventional terms. Working memory was the notable exception, where the effects of exercise were less consistent. The authors call for more rigorous trials with larger samples, appropriate controls and longer follow-up periods.

Where it fits

This analysis strengthened the case that aerobic exercise benefits cognition, because it drew only on randomised trials rather than the weaker designs that had fed earlier claims. At the same time it deflated the size of the promise: the gains are real but modest, a fraction of what enthusiastic coverage often implies. The inconsistency in working memory shows the benefit is not uniform across mental abilities. How long the gains last, and who benefits most, remained open questions for future trials.

What it means for you

This is a reason to count brain function among the returns on aerobic exercise, alongside the cardiovascular ones, and the evidence applies to adults broadly rather than only to older people. It is equally a reason for calibrated expectations: cardio nudges attention, processing speed, executive function and memory rather than transforming them. The trials involved supervised training sustained for longer than a month, so the evidence speaks to consistent habits rather than single sessions.
