Why it matters

Aerobic exercise is one of the few interventions with credible evidence for protecting thinking skills as people age, but trials rarely ask whether it works equally well for everyone. Women face a higher lifetime risk of Alzheimer's disease than men, and the hormonal transition of menopause is one plausible reason the same dose of exercise might land differently at different life stages. What they did

Researchers re-analysed a randomised controlled trial of 93 cognitively healthy women aged 20 to 67, of whom 43% were post-menopausal. Participants were assigned to either aerobic exercise or a stretching and toning programme, four days a week for six months. Neuropsychological assessment, a cardiorespiratory exercise test and a blood draw were performed at baseline, three months and six months. Linear mixed-effects regression models tested whether menopausal status moderated the effect of exercise type on executive functions and processing speed, controlling for age, education and baseline cognitive performance.

What they found

Seventy-six participants, or 81.7% of those enrolled, completed the intervention. A three-way interaction between time, group and menopausal status emerged at three months and persisted at six months. The pattern behind it was that post-menopausal women in the aerobic arm improved more in executive function than both the stretching and toning group and pre-menopausal women. The models adjusted for age, education and baseline scores, which matters because age and menopausal status are entangled. The abstract reports the executive function interaction rather than a comparable effect for processing speed.

Where it fits

This adds nuance to the broad finding that aerobic training benefits cognition by suggesting the benefit may not be uniform across a woman's life course. Because it is a post hoc analysis of a trial designed for other purposes, the menopause comparison was not randomised and the subgroups are small, which is exactly the setting in which interaction effects most often fail to replicate. Whether hormonal change, age, or something else drives the pattern is untested here. A trial recruiting deliberately across menopausal status would be needed to confirm it.
