Why it matters

The hippocampus sits at the centre of memory, and it is one of the most closely watched brain regions in ageing research. A widely repeated idea holds that aerobic exercise can physically enlarge it in later life, which would make cardio a direct tool against age-related brain change. Individual trials have pointed in different directions, so pooling them is the honest way to ask whether the effect is real. This analysis set out to test whether supervised aerobic training changes hippocampal volume in older adults with no cognitive impairment, and whether fitness gains track any brain change.

What they did

The researchers pooled eight randomised controlled trials covering 554 cognitively unimpaired, healthy older adults. Every programme delivered fully supervised aerobic training lasting between 3 and 12 months, averaging 129.85 minutes per week at moderate to vigorous intensity. Hippocampal volume changes were compared between exercise and control groups, and cardiorespiratory fitness was measured alongside. A further analysis then tested whether improvements in fitness were associated with changes in hippocampal volume.

What they found

Aerobic training produced no significant effect on hippocampal volume; the pooled estimate leaned very slightly positive but did not reach significance. Cardiorespiratory fitness, by contrast, improved moderately in the exercise groups. Crucially, gaining fitness was not associated with any change in hippocampal volume, which undermines the idea that a fitter heart translates directly into a bigger memory centre. The authors caution that methodological limitations across the included trials could be muddying the picture.

Where it fits

This result complicates one of the most cited claims in exercise neuroscience: that aerobic training can grow the ageing hippocampus. With only eight eligible trials, the evidence base is far thinner than the claim's popularity suggests, and the authors are explicit that the number of studies is limited. Open questions remain about longer or more intense programmes, other forms of exercise, and whether people with early cognitive decline respond differently. It also leaves untouched the separate question of whether exercise benefits memory performance rather than brain structure.

What it means for you

If you do cardio partly because you have heard it physically grows the brain's memory hub, this pooled evidence does not currently support that specific claim in healthy older adults. The fitness benefit is not in doubt here: training reliably improved cardiorespiratory fitness, which carries its own value. Brain volume is only one measure, and this analysis says nothing about thinking, memory or dementia risk. It is a reason to hold the brain-growth story more loosely, not a reason to stop moving.
